This request page is applicable only for residents of the state of California. If you are a California resident and wish to contact Elevate regarding a data privacy request, please submit a privacy request to either request to know your information (specific categories or pieces of information) to correct your information or to delete your information.
- Sign in below with your Elevate username and password or we’ll ask you for some information to help us identify you.
- Tell us what kind of request you’d like to make (Request to Know, Request to Correct, or Request to Delete).
- We will contact you within 45 days of the date of your request.
Please note the California Consumer Privacy Act only applies to California residents. If you are not a resident of California, we will not accept your request
